Some students may require a state-issued accessible parking placard in order to park in designated accessible spaces on campus. In Missouri, this requires completion of Department of Revenue (DOR) Form 1776 by a licensed medical provider.

Student Health Services can provide medical evaluations for this documentation on a case-by-case basis, when clinically appropriate.

You may consider requesting an evaluation if:

  • You do not currently have a state-issued accessible parking placard, and
  • You believe you meet Missouri’s medical eligibility criteria for a disability placard.

Students who already have a valid state-issued placard (including out-of-state placards) do not need additional medical documentation from Student Health.

Important Information About Eligibility

  • A provider must examine the patient before completing Form 1776.
  • If you are currently under the care of an outside specialist, you must request documentation directly from that provider. The form does not need to be completed by a physician in Missouri
  • If Student Health has previously evaluated or referred you for your condition and has appropriate documentation on file, our clinicians may be able to complete the form.
  • Completion of Form 1776 is based on clinical assessment and medical judgment.

Student Health does not determine final eligibility for a placard — the Missouri Department of Revenue makes that determination.
