As a patient, you have rights and responsibilities regarding your treatment and interaction with Student Health Center professionals.

Providers at the Student Health Center oppose all discrimination in any form and work to foster an inclusive environment in which to deliver needed health care.

A designated family/support person may be present to support the patient during their visit.

The Student Health Center does not provide written notes for missed classes, examinations, extracurricular activities or employment.

At the conclusion of a Student Health Center visit, you’re given a receipt confirming the date and time of your visit.

The release of information form authorizes Health Services to use and disclose protected health information.
