For any student under the age of 18, we must obtain consent for medical treatment prior to any visit.

All students must provide health history, immunizations and other information.

The release of information form authorizes the Student Health Center to use and disclose protected health information.

If you prefer to use email for personal health information, you can complete an authorization form.
